[cod] Serious bug: No die handler for player entity type 15

Further investigations have shown that the error IMHO occurs when a 
player does the following:
- Wait until Map changes.
- Wait some seconds (usually about 30 seonds after map change).
- Change the team to allies and immediately to axis. This must be done 
within 1-2 seconds. There is a key binding for immediately team change I 
do not publish here.

Maybe the player did something during the time from JT to D - I don't know.

After that the player was invulnerable / immortal / invincible.

game or the map changed.

While the player was attacked the console noted "No die handler for 
player entity type 15".

Our Server-Settings (regarding timers):

Many players (>10) are required for this to work, Game mode TDM.

I really don't know what is required for this bug to appear nor what 
exactly the reason is. I only noted what our log shows and what I think 
about it.
