Spark of death

Hey all,
   Zakk overheard me moaning about the spark of death, and suggested I fix
it.  I've got some time on my hands, so I'll take a stab at it.  Does
anybody have any hints as to what's causing it, or how to replicate it, or
anything else that could nudge me in the right direction?
