Improved CDROM code in CVS

I just committed some enhancements to the CDROM detection code in
icculus.org loki_setup CVS.

I have also included a new 'testcd' program to easily test the new code
on various platforms. Just do a 'make testcd' to compile it as it is not
built by default.

I have tested it on several Linux distros, as well as a number of Unix
platforms and it should be working pretty well now. Of course, please
try it and let me know if it solves the recent issues we had.
