[cod] tdm fails to rotate map on occasion

This is fine. The reason you need to specify a gametype before each map is
if you enable call voting and the gametype of say a tdm rotation get changed
to sd, it will continue on as sd. If you don't allow call voting it isn't an
issue.

Weird doesn't happen to me, although I set the gametype prior to
sv_maprotation and then each time the gametype changes.  
So in my config I have:
set g_gametype "sd" //gametype: bel, dm, re, sd, tdm

above my maprotation:

set sv_mapRotation "gametype sd map mp_brecourt map mp_carentan map
mp_hurtgen map mp_rocket gametype re map mp_powcamp map mp_pavlov map
mp_depot gametype tdm map mp_dawnville map mp_harbor map mp_railyard map
mp_ship map mp_chateau"

No hangs, no crashes. I also use the +map_rotate in the command line.

I have found that if you don't put a gametype for each map, the server
will hang.

If you don't mind me asking.  Why do you have so many gametype tdm's in
there if you aren't actually changing gametypes?  If you are going to
run
all tdm you should only have to put it in once in the
sv_maprotation.....

map mp_brecourt

What this does is tell CoD that the first map is Brecourt and your map
rotation tells it the game type.

This issue will magically dissappear?  I'm aprehensive to add "map 
mp_brecourt" w/out "gametype tdm", I guess it can't hurt (much).
