The failure to recognize buttons should be fixed by the
"more-joystick-buttons" branch in my Aquaria repository
(http://achurch.org/cgi-bin/hg/aquaria/); if you clone that and do
"hg export more-joystick-buttons", you should get a patch that applies
more or less cleanly to the current icculus.org sources.

For the right stick, make sure you have "Stick 2 X" set to 2 in the Key
Config menu (the default is 4).  Also keep in mind that the right stick
is only used for looking around (as in when the middle mouse button is
pressed) and for aiming.

If you're curious, here are the settings I use for a PS3 controller
(everything not listed is unset):

Left Mouse Equiv.   Button 14 (Cross)
Right Mouse Equiv.  Button 13 (Circle)
Revert              Button 15 (Square)
World Map/Recipes   Button 12 (Triangle)
Menu/Escape         Button 3 (Start)
Stick 1 X: 0 / Stick 1 Y: 1 / Stick 2 X: 2 / Stick 2 Y: 3
Prev Page           Button 8 (L2)
Next Page           Button 9 (R2)
Food Menu Cook      Button 0 (Select)
Food Left           Button 10 (L1)
Food Right          Button 11 (R1)
Look                Button 2 (R3 = press in the right stick)
