Bonus handling updates in CVS

I made minor bonus handling updates to CVS:
- fix bonus timing & group
- bonusmachine can now create objects if bonus asks for that
  (through variable in a global global_bonus struct)

I'm going to have a bit bit of a fun with latter next:
You get a "bird" bonus.  A few seconds later "bonusmachine" spits
out a bird.  It starts to follow you, and stop that only when:
- it dies
- you die
- you get "bird" bonus again

(I didn't add it yet but it's fairly trivial to do now)
