MD4 Model Support

To the best of my knowledge none of the MD4 stuff is used. There
certainly isn't any interface available to cgame at any rate, so it
isn't possible to use it from a mod. MD4 was something that was promised
to the mod community in "the next patch" for a couple of iterations, but
it never quite materialised. The implementation that is there is most
likely throughly unfinished and/or cruft.
