i had exact same problem. i set my MAXRATE to 10000 from 7000. this stopped the problem. i think most
ppl get the slow server msg still even tho the server runs smooth. whether this will remedy your problem i 
cant say.

  Our server lags like hell.. well not all the time but it peaks alot! 
  When i play my ping is 30,40,50.. but it goes up to 130 or more for a second and then it's normal again.

  Do you know the problem ? I set the max ping to 500 but that's only on connect, i have set autokick to 150.
  But the strange part is when we play with 16 people it lags and when we play with 4 it lags.

  Maybe a fres server install ? Or is it purely the server... ?
