I've updated "Appendix F: Default Key Shortcuts" and "Appendix G: Shortcut keys
and Mouse functions in GtkRadiant" to reflect GtkRadiant 1.4 commands and
shortcuts. I removed all the QE/Q3Radiant stuff. If people want documentation
for the old editor, they should get the old documentation, IMO, and I didn't
want to try to document all the versions of GtkRadiant from 1.1 to 1.4. 

Also fixed a navigation link in appendix E.

I'll try to update other stuff as time permits.
