Patch to remove old entity plugin stuff:

This diff removes (by default) the exisiting PluginEntities code by way of
#ifdef's.  If you need to compile with PluginEntities then compile RADIANT and
all of its MODULES with a preprocessor defintion of "USEPLUGINENTITIES"

Why:

This is being done as the plugin entity's naming convention interferes with
moving the entity code into a seperate module.

It can be added back in later with minimal fuss if required, according to
ttimo the functionality this code provided was only used internally by EA.
There are also a few FIXME's and TODO's in the original code so it looks like
it was never fully completed.
