<TTimo> sanitizing the IncRef stuff becomes pretty urgent .. I had started it in
synapse, but it's probably better to merge it in independantly asap
<TTimo> and throw in a qrefcount.h header

The main point about this is to document a standard way of doing reference
counting, and to provide useful macros. One of the features to explore would be
a 'debug mode', checking that all references are properly deleted etc.
