If so then these are the most important parts of what glxinfo should be 
something like this.

Certainly if any of the places where it says NVIDIA say something else then 
there is a problem.

Hmmm, odd that.  What happens when you run UT2003?  Does it crash out 
immediately?  If so, and you're using a 2.4 kernel, you might want to try 
removing the TLS verions of NVIDIA libraries.  These are installed 
in /usr/lib/tls.

You might want to upgrade to 5336.  This is also the first version to 
officially support the 2.6 kernel series.
