Log message:

Killed gl_fb_models and gl_fb_bmodels cvars.
Replaced them with gl_fb.

Kill the top_bottom palette, it was an evil hack.

Rework how to handle indices for alias models.

Fix some rendering errors with the above.
(We were not drawing some models, for instance.)
