Minor cleanups I've been sitting on - I intended to do other cleanups
first, but these will have to suffice for now.  Cvar _snd_mixahead has had
its default cut from 0.1 to 0.05 as well.  It makes for more responsive
audio since we've got the sound painting thread.  This value is likely not
suitable for systems without multithreading such as some old UNIX systems
and MacOS Classic, but to my knowledge there is nobody even working
support for such systems at this time.
