[sdlsound] can't stop coding

Hi,

added some documentation, cleaned up the code a little
- and missed some of the changes of Ryan. Ryan, can 
you took a quick glance about the docu (I'm not a native
speaker, and my english tends to be in a bad shape)
and reapply your latest change?
Torbjorn, I hope the documentation explains the calculation
done in the rate converter. Don't bother to ask if something 
left in the dark.
