[q2x] Refactorization

Baby steps.

First pass is that I've renamed "qshared/" to "qutil/", adjusted 
DSP/DSW files to work (makefiles are not updated).  Also renamed a 
couple other files.

Otherwise should still work.

So now we have:

server/
client/
game/
qutil/
qcommon/

As our current major subsystems.

I still really want to go through almost line by line and clean up 
huge scads of this shit before doing the extensions...
