e: [openbox] Extended busy cursor when launching certain apps from konqueror

Solved.  It turns out that in addition the the general settings for the
cursor in the control panel, there is also a setting in the "advanced"
section for each specific application under "Edit file type" in the
konqueror menu that enables startup notification by default.
Once this is disable the problem no longer occurs.
