updated polygon code from darkplaces codebase, this fixes a bug in
PolygonF_QuadForPlane and PolygonD_QuadForPlane that existed when the
last copy from darkplaces code occurred, and fixes a double/float
conversion warning, as well as improving PolygonF_Divide/PolygonD_Divide
to tell the caller the number of on-plane points, and some general code
cleanup as well
