Log message:

added FS_Rescan() call to Resource_UnloadAll
added "resource_flush" console command (calls Resource_UnloadAll)
removed RESOURCEFLAG_ALLOWFAIL because it simply couldn't work correctly
restructured Resource_IndexForName to make it call Resource_Load if RESOURCEFLAG_PRECACHE is set if the resource entry already exists but is not loaded (previously it only did this if the resource entry did not exist)
