This is what my top prints out with 26 clients in the server....

My spearhead eating 90% of my cpu and its only 7.9% idle
My box specs are:
Intel 2.4Ghz
512Mb RAM
100Mbps Pipe
sv_maxrate "10000"

The only tip i know is maxrate, also, try to set high number of private
slots so it consumes less cpu most of the time when the server is "full"
