checking in patch 84 + modifs

the texturing code still has a bunch of issues

- added some items in the todo about it
(see http://zerowing.idsoftware.com/doxygen/html/todo.html)

- the loading of patches is screwed, they all show up with no texture (try
q3dm7sample)
