Dedicated Server Problems

As the list is already being (ab)used for codwwbeta windows dedicated server questions ;), has any of your guys experienced problems with dedicated servers? 
Mine are regularly hanging up on "ERROR: G_Spawn: no free entities", I tried to change few configuration settings but it just keeps happening random during the game. 

When I manually perform a "map_rotate" command, the server keeps running.
