Here's PB the man page for COD2 as EB doesn't have the COD4 one up, but PB
commands are the same across all PB enabled games except for AA.

To ban the player using PB simply find the pbbans.dat file open it with a
text editor.
Add the GUID from the screen shot to the bottom of the file following the
format shown from the previous lines in the file.

Or use this rcon command to do it.

/rcon PB_SV_BanGuid 0b7c4147e9a9528d2906fe75587a8af7 DR.Zero

By the way if you were streaming to a site like www.punksbusted.com you
could submit the screen shot and get said player banned from quite a few
COD4 servers permanently.

I tried to ban the guid but banuser and banclient need the player to be 
banned on the server, but he was leaving before. Is there a way to ban 
players which are not on the server? Is there a way to add a reason for 
kicks and bans which can be seen by the kicked/banned player?
