Random strangeness on serve

We've got one customer saying they have players:
1. spawning without weapons
2. spawning invisible to other players
3. CTF timer getting stuck on 0.00 so the next map
never starts
4. HQ the radio sometimes never spawns.

Anyone confirm or deny this apparently the invis is not
a cheat as the owner has had hit happen to them.
