It happened to us with 1.41d as well as 1.41e

We are getting a routine crash with mp_kharkov and 1.41e.  It is happening
on
all servers:

ERROR: G_Spawn: no free entities

I know much has been said about maps causing this, but it did not happen
with
1.41d.  We are running RH9 on dual 2.8 XEON processors.
