Just a quick questiojn about the location of this server, is it at home? IF 
yes then is the your firewall setup for forwarding properly?

I think if the sv_dedicated was set to 2 like it should be in this case then 
you should be seeing the heartbeat output in the server sonsole

do you have +set dedicated 2, in your startup?  This makes a server
send a heartbeat to whichever servers you have specificed in your
sv_master settings.  If you dont have sv_master settings set, it will
default to the one that is needed for you.  Connect to your game
server and from console or using rcon, just type /dedicated <ENTER>
if it comes back with 1, then its set up for LAN play, disallowing
heartbeats to the master server.  However, if you have punkbuster
enabled, I dont think the dedicated setting will matter as punkbuster
will do its think.  iF you dont knwo if punkbuster is on or not, from
console or rcon you can issue '/sv_punkbuster <ENTER>' and see.  You
cant just turn it on at will though.  You should have +set
sv_punkbuster 1 set in your launch line also, so an example would be

./cod_lnxded +set dedicated 2 +set sv_punkbuster 1 +exec
yourserverconfig.cfg +map_rotate

at least in the simplest form, hope this helps.
